WDFW recommends downloading and filling out the "Hydraulic Analysis Spreadsheet" then forwarding to WDFW with the Site Description Field Form and Level A Culvert Assessment Field Form. WDFW will complete the analysis and send you the results.

WDFW needs you to complete the first two bottom tabs, the ones called "Culvert Data" and "Channel Data". WDFW will complete the third tab "QFP Calculator".

Level B Spreadsheet & Supporting Photos Upload*
Please upload the completed Level B Spreadsheet.xls in addition to your photos. Appendix F of the Fish Passage Inventory, Assessment, and Prioritization Manual (PDF) covers the importance of taking and submitting photos. It’s only 2 pages - F1 and F2, but WDFW is big on pictures. HOW TO NAME PHOTOS: Label with site ID, followed by a number based on whether it is an inlet, outlet, or habitat photo. SITEID_1 for outlet SITEID_2 for inlet SITEID_3-AND UP for habitat photos. The order is based on how fish encounter the specific culvert. They see the outlet first so it's #1 and so on. One key thing to note, if you have multiple photos of an outlet or inlet, pick the best one that shows the culvert the best. WDFW told me they just want one photo of the outlet and one of the inlet. It is important when taking photos of the culverts to get back a good distance so that WDFW can get a good idea of what the road fill and surrounding area of the culvert looks like. If your phone camera has a wide angle lense, use it. Do not submit multiple photos online of one outlet or one inlet. HOWEVER, you can submit as many habitat photos as you want (SITEID_3, SITEID_4, SITEID_5, etc.).
